Five Treatment Paths—Plus One Emerging Area Under Review.

Patients often arrive knowing they want an injection but not knowing which type. These pages explain the major options, plus one emerging area we are actively reviewing as the evidence develops.

Hyaluronic Acid / Gel Injections

Gel injections, also called hyaluronic acid injections or viscosupplementation, are used primarily for symptoms related to knee osteoarthritis. They are different from cortisone and different from biologic treatments such as PRP.

Common reason patients ask about them: knee arthritis and trying another nonsurgical option before considering surgery.

PRP Injections

Platelet-rich plasma (PRP) is prepared from a patient's own blood. It concentrates platelets and biologically active components and may be considered for selected arthritis, tendon and sports-medicine conditions.

Common reason patients ask about it: selected joint or tendon problems when they want to understand biologic treatment options.

Stem Cell & Orthobiologic Treatments

“Stem cell therapy” is a broad term used for several different tissue-based treatments. The science and terminology can be confusing, so this page focuses on what these treatments actually are, what is still being studied and what patients should understand before paying for one.

Common reason patients ask about them: arthritis or sports injuries after hearing claims about “regeneration” or avoiding surgery.

Cartilage Repair & Restoration

A focal cartilage defect is not the same thing as generalized osteoarthritis. Selected patients—often younger or more active—may have joint-preservation options designed specifically around a localized area of cartilage damage.

Common reason patients ask about it: a focal cartilage injury, osteochondral defect or persistent symptoms despite nonsurgical care.

Amniotic Tissue Injections

Amniotic and other birth-tissue-derived products are sometimes marketed as regenerative injections for arthritis and soft-tissue problems. Products in this category can differ substantially in composition, processing, evidence and regulatory status, so patients should understand exactly what product is being proposed and what data support its use.

Common reason patients ask about them: arthritis or tendon pain after hearing about amniotic, placental or “birth tissue” regenerative injections.

BPC-157 and other peptides are receiving attention for possible musculoskeletal healing. Human orthopedic data remain limited, and we do not currently offer peptide therapy. We are following the evidence as higher-quality research becomes available.

The Most Important Step

The Diagnosis Matters More Than the Injection.

Knee, shoulder and hip pain can come from very different problems. Treating every painful joint with the same injection does not make sense. The right treatment depends on what is actually damaged, how advanced the problem is, your goals and what has already been tried.

Start With an Orthopedic Evaluation

Identify the source of pain

History, examination and appropriate imaging help distinguish arthritis, tendon disease, cartilage injury and other causes.

Understand the severity

Mild or focal disease may have very different options from advanced structural arthritis.

Choose the simplest useful next step

Sometimes that is therapy or medication. Sometimes it is an injection. Sometimes another treatment makes more sense.

Reassess when the plan stops working

The goal is not to keep repeating procedures indefinitely; it is to move treatment forward when symptoms or disease progression warrant it.

Cortisone vs. Gel vs. PRP vs. Orthobiologics

These treatments are not interchangeable. This quick comparison is a starting point—not a substitute for examining the joint and making a diagnosis.

Insurance coverage, out-of-pocket cost, indications and availability vary by treatment, diagnosis, payer and individual clinical circumstances.

A Practical Approach

What We Mean by “Regenerative Medicine”

The term is used loosely online. We prefer to be specific about what is being injected, what problem it is intended to treat and what the evidence can reasonably support.

No One Injection Fits Everyone

Age, diagnosis, severity, activity level, prior treatment and patient goals all matter when deciding whether an injection is worth considering.

Symptom Improvement Is Not the Same as Regrowing a Joint

A treatment may help pain or function without recreating normal cartilage. Patients deserve to understand that distinction before choosing a procedure.

Surgery Is Not Automatically the Answer Either

Some patients can remain active with nonsurgical treatment. Others may eventually be better served by cartilage restoration or joint replacement when structural disease is advanced.

Emerging Treatments

What About BPC-157 and Peptides?

New treatments deserve interest—but they also deserve the same standard of evidence we apply to everything else in medicine.

Promising ideas are not the same as proven orthopedic treatments.

BPC-157 and other peptides are receiving significant attention for possible effects on tendon, ligament, muscle and other tissue healing. There are interesting preclinical data, but human orthopedic evidence remains very limited.

As physicians, we have to evaluate more than whether a treatment sounds biologically promising. We need reliable human data on safety, dosing, product quality, effectiveness and which patients—if any—actually benefit.

Our current position: BPC-157 remains investigational and is not FDA-approved. At this time, we do not believe there is enough high-quality human evidence to offer BPC-157 in our office. We will continue to follow the research as better data become available.

Image-Guided Procedures

Ultrasound Guidance When It Adds Value

Many injections can be performed accurately using anatomic landmarks. For selected deep, small or difficult targets, point-of-care ultrasound allows the physician to visualize the needle and intended structure in real time.

Why use ultrasound? It can provide another set of eyes in the room when real-time imaging will meaningfully improve the evaluation or procedure. It is used selectively—not automatically.

Joint Injection & Regenerative Medicine FAQs

Straight answers to the questions patients commonly ask before choosing between gel, PRP, cortisone, amniotic tissue and other orthobiologic treatments.

What is the best injection for knee arthritis?

There is no single best injection for every knee. The choice depends on the degree of arthritis, symptoms, prior treatment, age, activity level, medical history and goals. Cortisone, gel injections and PRP are different treatments with different strengths and limitations.

Is PRP better than gel injections?

Not for every patient. Gel injections are used primarily for knee osteoarthritis, while PRP may be discussed for selected arthritis and tendon conditions. The more useful question is which treatment fits the specific diagnosis and stage of disease.

Is PRP the same thing as stem cell therapy?

No. PRP is prepared from a patient's blood and concentrates platelets and associated biologically active components. Tissue- or marrow-based orthobiologic treatments are different procedures.

Can regenerative medicine regrow cartilage?

Patients should be cautious with claims that an injection can reliably recreate normal cartilage or reverse advanced arthritis. Some biologic treatments are studied for symptom and functional improvement, but that is different from guaranteeing restoration of a normal joint.

Can injections help me avoid joint replacement?

Some patients can manage symptoms for extended periods without surgery, especially when arthritis is not advanced. Injections do not reliably reverse severe structural arthritis, so joint replacement may eventually become the more appropriate treatment for some patients.

What are amniotic tissue injections?

These are products derived from donated birth tissues and marketed in several different forms. Their composition, processing, biologic activity and regulatory status can vary substantially. No amniotic product is FDA-approved to treat an orthopedic condition, so patients should understand the specific product and evidence before considering treatment.

Does Atlanta Bone & Joint offer BPC-157 peptide injections?

Not at this time. BPC-157 has interesting preclinical data, but human orthopedic evidence remains limited and it is not FDA-approved. We believe additional high-quality human data on safety, dosing, product quality and effectiveness are needed before offering it in our office.

Do I need an MRI before getting a joint injection?

Not always. A history, physical examination and X-rays often provide substantial information. MRI or ultrasound may be added when the additional imaging is likely to change the diagnosis or treatment plan.
