Amniotic tissue products have received attention in regenerative medicine because of their biologic properties and their use in certain wound-healing and reconstructive settings. Their role as an injection for arthritis or orthopedic injuries is much less established. We think patients should understand that difference.
Much of the interest in amniotic tissue comes from a longer history of using or studying amniotic membrane as a biologic covering in wound care, burns, ophthalmology and reconstructive medicine. Injecting a processed amniotic or birth-tissue product into an arthritic joint is a different use with a different evidence base.
Amniotic membrane has been studied as a biologic dressing or graft for difficult wounds and tissue surfaces. Plastic and reconstructive surgeons, wound-care specialists and other physicians have used or investigated these materials because they can provide a biologic scaffold and may influence inflammation and healing at the wound surface.
Some clinical studies of specific amniotic suspension products for knee osteoarthritis have reported improvement in pain and function. That is encouraging, but the number of high-quality human studies is still limited, products are not interchangeable, and symptom improvement is not the same as proving cartilage regeneration.
At Atlanta Bone & Joint Specialists, we consider amniotic tissue injections an investigational orthopedic treatment. We do not market them as a proven way to regrow cartilage, reverse arthritis or replace surgery.
We will continue to review the orthopedic literature as better studies become available and change our recommendations when the evidence justifies it.
A focal cartilage injury, mild arthritis, advanced bone-on-bone arthritis and a tendon injury are different problems and should not automatically receive the same treatment.
Patients should know what tissue or material is being injected, how it is processed and whether the evidence being discussed actually applies to that specific product.
Improvement in pain is meaningful, but it is different from proving that an injection regenerates normal cartilage or changes the long-term course of arthritis.
Depending on the diagnosis, other injection or joint-preservation treatments may have more established evidence or may simply fit the problem better.
Short answers to the questions we think patients should ask before considering any birth-tissue or regenerative injection.
They should not automatically be described that way. Products marketed as amniotic, placental or birth-tissue treatments can differ substantially in composition and processing. Patients should ask exactly what the product contains rather than relying on the phrase “stem cells.”
There are human studies—including randomized trials of specific amniotic suspension products—reporting improvement in knee arthritis symptoms. The overall evidence remains limited compared with more established orthopedic treatments, and results from one product cannot automatically be generalized to every amniotic product.
That has not been reliably demonstrated in humans. A treatment may improve pain or function without restoring normal cartilage. We would be cautious with any claim that an amniotic injection can predictably reverse advanced arthritis.
Amniotic membrane can act as a biologic tissue covering or scaffold and has been studied in several wound-healing and reconstructive settings. That application is different from injecting processed tissue into a joint, so wound-healing experience should not be treated as proof that a joint injection works.
No regenerative medicine product in this category is FDA-approved as a treatment for orthopedic conditions such as osteoarthritis, knee pain, hip pain or shoulder pain. Regulatory status can also depend on the specific product and how it is processed and used.
No. We currently view these injections as investigational in orthopedics. We are following the research and believe recommendations should change when stronger human evidence demonstrates which products are safe, effective and appropriate for specific orthopedic diagnoses.
